Chelan County
In June 2003, the Nursing Commission charged registered nurse Regina R. Wilkins (RN00035950) with
unprofessional conduct. Allegations against Wilkins include failing to transcribe physician orders,
failing to assess a patient’s condition and failing to accurately and completely document care.

Clark County
In June 2003, the Nursing Commission charged Thea C. Lewis, a licensed practical nurse (LP00035608),
with unprofessional conduct. Lewis allegedly failed to comply with the terms and conditions of a
prior order.

In June 2003, the Nursing Assistant Program charged Adam L. Shipman, a nursing assistant
(NC10056468), with unprofessional conduct. Shipman allegedly verbally and/or physically abused
several patients. He was found guilty of assault in relation to this conduct.

Cowlitz County
In June 2003, the Pharmacy Board charged Monica L. Roe, a pharmacy technician (VA00042511), with
unprofessional conduct. Roe allegedly diverted medications from her place of employment.

Franklin County
In June 2003, the Nursing Commission reinstated the license of Richard L. Huntley to practice as a
licensed practical nurse (LP00034678). His license is subject to probation and he must complete a
refresher course and seek a substance abuse evaluation.

Grays Harbor County
In June 2003, the Nursing Assistant Program charged Michael Latham, a nursing assistant
(NC10061037), with unprofessional conduct. Latham was allegedly convicted of shoplifting.

Island County
In June 2003, the Nursing Commission charged registered nurse Roger W. Bennett (RN00125333) with
unprofessional conduct. Bennett allegedly took a pad of blank prescription forms belonging to a
physician. He used the prescription pad to forge prescriptions for his brother.

King County
In June 2003, the Nursing Assistant Program charged Cristina Antemie, a nursing assistant
(NA00054401), with unprofessional conduct. In August 2002, the Department of Social and Health
Services revoked her facility license. Antemie also allegedly obtained controlled substances with
forged prescriptions.

In June 2003, the Nursing Commission placed the license of Garry N. Bice, a licensed practical nurse
(LP00047153), on probation for one year. He must complete 16 hours of course work and pay a fine of
$250. Bice failed to document an encounter with a patient and provided patient instructions and/or
information for which he lacked appropriate experience and knowledge.

In June 2003, the Nursing Assistant Program charged Judith L. Gideon, a nursing assistant
(NC10049542), with unprofessional conduct. Allegations against Gideon include sleeping and appearing
intoxicated while on the job.

In June 2003, the Dental Commission entered into an Agreed Order with dentist Robert M. Keenan
(DE00003579). Keenan agrees to complete continuing education and pay a $7,500 fine. He provided
services to a patient that failed to meet the accepted standard of practice.

In June 2003, the Dental Commission placed the license of dentist Hoi Lee (DE00004763) on probation
for two years. He must pay a $7,500 fine and complete continuing education. During an infection
control inspection, Lee’s place of practice had several deficiencies. These include an unclean work
areas and Lee wearing contaminated gloves when touching patients.

In June 2003, the Nursing Assistant Program charged Charlene D. Manley, a nursing assistant
(NA00146468), with unprofessional conduct. Manley allegedly diverted controlled substances from her
place of employment.

In June 2003, the Medical Commission, on remand from the Washington State Supreme Court, affirmed
the September 1997 Final Order revoking the license of Bang D. Nguyen (MD00021157). Since the
five-year revocation period imposed in September 1997 has already expired, Nguyen may reapply for
licensure. He shall also be required to demonstrate that he is competent and can safely practice as
a physician. He shall be required to comply with the laws concerning application, eligibility and
examinations.

In June 2003, the Pharmacy Board charged Penny Rodwell, a pharmacy technician (VA00016096), with
unprofessional conduct. Rodwell allegedly admitted to diverting drugs from her place of employment.

In June 2003, the Nursing Commission withdrew a statement of charges against registered nurse
Russell L. Stevens (RN00133989).

In June 2003, the Medical Commission charged H. Richard Winn, a physician and surgeon (MD000121090),
with unprofessional conduct. Winn plead guilty to obstructing a criminal investigation of health
care offenses. He also admitted to lying to, persuading and intimidating other physicians and
employees.

Pierce County
In June 2003, the Nursing Commission charged Janis D. Mc Caughan, a licensed practical nurse
(LP00047259), with unprofessional conduct. She allegedly inappropriately delegated the
administration of a patient’s medications to a nursing assistant, failed to assess a patient and
failed to adequately secure medications.

In June 2003, the Nursing Commission placed the license of registered nurse Mary Jo Warren
(RN00080398) on probation for one year. She must pay a $250 fine and complete 18 hours of
coursework. Warren failed to adequately place a patient on "alert status" and failed to timely
assess and treat a patient.

Skagit County
In June 2003, the Pharmacy Board charged Ofelia Polanco, a pharmacy technician (VA00048448), with
unprofessional conduct. Polanco allegedly admitted to diverting controlled substances from her place
of employment to sell and/or trade for food stamps. As a result, Polanco was charged with theft and
sale of a controlled substance for profit in Skagit County Superior Court.

Snohomish County
In June 2003, the Nursing Commission fully reinstated the license of Roseanne M. Peters to practice
as a registered nurse (RN00090794).

Spokane County
In June 2003, the Nursing Assistant Program charged Randy R. Ferguson, a nursing assistant
(NC10048774), with unprofessional conduct. Ferguson allegedly failed to comply with the terms and
conditions of a prior order.

Stevens County
In June 2003, the Respiratory Care Program charged Blaise O. Cordero, a respiratory care
practitioner (LR00002101), with unprofessional conduct. Cordero was allegedly convicted of
voyeurism.

Thurston County
In June 2003, the Nursing Commission placed the license of registered nurse Karen L. Clanton
(RN00050447) on probation for two years. She must submit an evaluation report and personal progress
reports to the commission. Charges against Clanton include failing to notify the physician of a
patient’s condition, failed to administer physician’s orders and documented an assessment of a
patient when no assessment was made.

Out of State
(Many health care providers hold licenses in more than one state. Department of Health receives
information each month from national groups on disciplined health care providers. When another
state’s regulatory group takes actions, the equivalent board/commission/program in Washington files
unprofessional conduct charges against the provider’s Washington credential.)

Idaho: In June 2003, the Nursing Assistant Program charged Scott Giordano, an applicant for a
registration to practice as a nursing assistant in Washington (RA00026731), with unprofessional
conduct. Giordano was allegedly convicted of Arson in Idaho.
